Open House in the NEW Youth Center!

Parental Consent Forms:

This form needs to be completed, signed and returned to the church before participating in any youth activity that requires leaving the church premises.  We use this form because we want to make sure:

a) that parents or legal guardians know where their youth will be when they are in our custody, and

 

b) that parents or legal guardians are allowing their youth to leave the church premises for a specific youth activity.
